Chronic diseases are long-term conditions that require ongoing care, even when you feel well. Without proper management, these conditions can lead to complications that affect your health and daily life. In Singapore, around one in three residents has high blood pressure, and 9.1% have diabetes.

The Chronic Disease Management Programme (CDMP) helps manage chronic disease costs by allowing you to use MediSave for eligible outpatient treatment. Subsidies are also available if you qualify, supporting your ongoing care at participating GP clinics, including Healthway Medical.

What Is the Chronic Disease Management Programme (CDMP)?

The Chronic Disease Management Programme is a national programme that supports ongoing care for selected chronic conditions through regular reviews and treatment. Depending on your needs, your care may include:

  • Consultations to review your condition and treatment
  • Prescribed medication
  • Tests to monitor your condition and check for complications
  • Advice on diet, physical activity and other lifestyle changes

You can receive CDMP care at participating GP clinics, including Healthway Medical, as well as polyclinics and participating specialist outpatient clinics.

MediSave and CHAS claims for these mental health conditions are available at our participating Mental Health GP Partner clinics: Healthway Medical (Sunshine Place), Healthway Medical (Bukit Batok West), Healthway Medical (Fajar), Healthway Medical (Novena Medical Centre).

For other eligible CDMP conditions, you may also use MediSave and CHAS support at Healthway Medical GP clinics, subject to the applicable scheme requirements.

Who Is Eligible?

If you have a condition covered under CDMP, your doctor will assess whether your treatment meets the programme’s requirements.

You do not need to apply separately to join CDMP. When you visit the clinic, let the team know that you would like to use MediSave or applicable subsidies. They will review your eligibility and guide you through the required authorisation process.

The financial support available depends on your residency status, subsidy eligibility and where you receive care. You do not need to qualify for CHAS subsidies to use MediSave for eligible CDMP treatment.

Financial Support for Your Chronic Care

You may be able to combine government subsidies with MediSave to manage your treatment costs. Subsidies reduce your bill first. MediSave can then help pay the remaining eligible amount, subject to withdrawal limits and any applicable cash co-payment.

CHAS, Pioneer Generation and Merdeka Generation Subsidies

If you are an eligible Singapore Citizen, you can receive subsidies for chronic care at participating CHAS GP clinics. Under the CHAS Chronic Tier, the maximum annual subsidies are:

CHAS Orange$200$320
CHAS Blue$320$500

Pioneer Generation$360$540

For these subsidies:

  • Simple means your doctor is managing one chronic condition without related complications.
  • Complex means your doctor is managing two or more chronic conditions, or a chronic condition with related complications.

Per-visit limits also apply. Your clinic can advise on the subsidy available for your visit.

CHAS Chronic Diseases

If you hold a CHAS, Pioneer Generation or Merdeka Generation card and receive chronic care at your enrolled Healthier SG GP clinic, you can choose between the CHAS Chronic Tier and Healthier SG Chronic Tier.

The Healthier SG Chronic Tier may offer more support if you have higher ongoing medication needs. Here is how the two frameworks differ:

CHAS Chronic TierHealthier SG Chronic Tier
Where can you use it?Participating CHAS GP clinicsYour enrolled Healthier SG GP clinic
How does it help?Subsidies for consultations, tests and medicines, capped at $135 for each visit for chronic care and $540 a year.Subsidies of up to 87.5% for selected chronic medicines, with no annual dollar cap on these medication subsidies. Subsidies for consultations, tests and other medicines are capped at $135 for each visit for chronic care visit and $360 a year.
When might it be suitable? If your medication needs are lower and CHAS subsidies adequately cover your care.If your medication needs are higher and your prescribed medicines are on the Healthier SG Medication List.

The amounts shown are the highest available. Your clinic can advise how much subsidy you qualify for based on your CHAS tier or PG/MG status and treatment needs.

Your clinic can help you choose the framework most suited for your needs.  Only one applies at each visit. If you switch between them, your remaining annual subsidy balance is recalculated to account for subsidies already used. For more details, read our guide to CHAS, Healthier SG and MediSave.

Using MediSave for the Remaining Eligible Bill

Singapore Citizens and Permanent Residents may use their MediSave for eligible CDMP treatment, subject to the applicable account and claim requirements.

Under MediSave500/700, the annual withdrawal limits in 2026 are:

Your treatment needsAnnual withdrawal limit
One CDMP condition without related complications$500
Two or more CDMP conditions, or one CDMP condition with related complications$700

Complications are additional health problems caused by a condition, such as kidney damage from diabetes. Your doctor will confirm which limit applies based on your treatment during the calendar year. The annual limit is shared across your eligible claims, rather than applying separately to each condition.

Selected vaccinations and preventive tests also share the applicable MediSave500/700 limit.

Do You Need to Pay Any Cash?

A 15% cash co-payment generally applies to your eligible CDMP bill after subsidies. MediSave can help pay the remaining eligible amount, subject to sufficient savings and your remaining annual withdrawal limit.

If you receive CDMP treatment at your enrolled Healthier SG clinic, this 15% cash co-payment requirement is waived. You may use MediSave to pay the eligible balance in full, up to your available savings and remaining withdrawal limit.

What Changes From 1 January 2027?

From 1 January 2027, MediSave500/700 will be renamed MediSave Chronic and Preventive Care, with higher annual withdrawal limits.

Annual withdrawal limitIn 2026From 1 January 2027
Basic limit$500$700
